AMBULATORY CARE PHARMACIST
Job Description
Full Time, Varied Shifts, M-F a combination of 1st & 2nd shift, rotating weekends
Minimum Age Requirement: 18 years of age or older
- JOB REQUIREMENTS
- Education
- Graduation from an accredited school of pharmacy with at least a Bachelor of Science in Pharmacy (BSPH) degree required. Must be licensed to practice pharmacy in the state of Arkansas, such licensure granted by the State Board of Pharmacy after passing the required examinations or by reciprocity based in passing such examinations in another state.
- Experience
- Preferred: Completion of a PGY2 residency in Ambulatory Care or relevant specialty area, Board certification (e.g., BCACP, BCPS, BCIDP, BCOP, etc.), Experience in ambulatory or specialty care settings
- Physical
- This is a safety sensitive position. Please see the St. Bernards Substance Abuse Policy for further information.
- Normal hospital environment. Close eye work. Hearing within normal range. Operates computer, typewriter, calculator. Finger dexterity. Eye-hand coordination. Frequent sitting, standing and walking. May lift and carry up to 35 lbs. Some exposure to chemical hazards when making chemotherapy.
- JOB SUMMARY
The Ambulatory Care Pharmacist in the Specialty Clinic provides advanced, patient-centered pharmaceutical care in collaboration with providers and interdisciplinary teams. This role focuses on chronic disease management, medication therapy optimization, patient education, and care coordination for patients with complex conditions commonly treated in outpatient clinic settings.
