Full Time, Day Shift, M-F 8a-4:30p
Minimum Age Requirement: 18 years of age or older
- JOB REQUIREMENTS
- Education
- Baccalaureate Degree in Social Work, Psychology, Sociology or other field related to Social Work. Degree requirements may be waived for appropriate experience in related field.
- Experience
- One-year experience in a health care setting or two-years of appropriate work as a Social Work Assistant. Demonstrates knowledge and skills necessary to provide care to and communicate with primarily the geriatric population. Able to assess data reflecting the patient status and interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
- Physical

- Frequent indoor and outdoor activities. Frequent sitting. Occasional standing and walking. Lift, carry, push, or pull up to 10 lbs. Must be able to adapt to the patient’s status and needs and staff needs. Possible infection from patients, auto accidents, unsafe neighborhoods. Must have reliable transportation and current driver’s license valid in the state of Arkansas and automobile liability insurance.
- JOB SUMMARY
- Assists patients, families and/or caregiver in understanding the significant social and emotional factors related to health problems. Participates in the development of the plan of treatment by working as a team member. Utilizes community resources, participates in discharge planning, in-service programs, and case conferences. Maintains appropriate records concerning services to those patients. Acts as a consultant to the agency personnel.
