TRANSITIONS OF CARE PHARMACIST
Job Description
Minimum Age Requirement: 18 years of age or older
- JOB REQUIREMENTS
- Education
Graduation from an accredited school of pharmacy with Doctor of Pharmacy (PharmD) degree required. Must be licensed or eligible for licensure to practice pharmacy in the state of Arkansas.
- Experience
Minimum three (3) years of pharmacy experience in direct patient care or PGY-1 Pharmacy Residency required.

Normal hospital environment. Close eye work. Hearing within normal range. Operates computer, typewriter, calculator. Finger dexterity. Eye-hand coordination. Frequent sitting, standing and walking. May lift and carry up to 35 lbs. Some exposure to chemical hazards when making chemotherapy.
- JOB SUMMARY
The Transitions of Care Pharmacist is responsible for clinical aspects of pharmaceutical patient care including interaction with patients, families, physicians, nurses, and other healthcare providers. As a key member of the treatment team, the Pharmacist will optimize medication therapy management to improve clinical and economic outcomes and ensure a patient’s pharmaceutical plan of care is fully implemented through transitions of care. Activities will encompass participation in admission medication reconciliation, discharge medication reconciliation, patient counseling/teaching and multidisciplinary care planning.
